Patient was apparently asymptomatic 1 month back, following which he developed productive cough, low grade fever for which he underwent sputum studies and tested positive for AFB bacilli and started ATT - HRZE regimen, 2 tab according to weight/PO/OD.


He developed generalized weakness and myalgia 15 days back.


10 days back, patient got up from bed and went to open the door and suddenly fell down, with no loss of consciousness and no froathing. Following which his brother got him up and since then Patient developed bowel and bladder incontinence.

No sensory impairment.

The relevant medical history of the patient has been shared in a comprehensive and systematic manner. Advice at discharge was mentioned which provided an insight on what can be done. No diagnostic uncertainty was present. There were no discussions/links regarding the line of treatment followed.

analyze the above linked patient data by first preparing a problem list for each patient (based on the shared data) and then discuss the diagnostic and therapeutic uncertainty around solving those problems. Also include the review of literature around sensitivity and specificity of the diagnostic interventions mentioned and same around efficacy of the therapeutic interventions mentioned for each patient.

It would be a better experience if it was offline. till now we have learned general examination and history taking.

We, the students of third semester just began our clinical postings for the first time through online learning system which is a difficult task compared to direct or the offline teaching.

 but we are trying our level best to present our logs. Interns, pg's and faculty of general medicine are helping us to understand the case during our online classes. The online classes are not perfect due to the connection issues and other problems but the faculty is trying their best to make us understand amidst the pandemic. As we recently entered our 3rd semester and postings started just 2-3 weeks ago we are having a bit difficulty to understand all the topics and comprehend them but we will try cope up with the subject as soon as possible. We are able to know different cases ,their symptoms and treatment. Discussing the topic with my friends made my work a bit easy.

This assignment has helped me to learn the basics of clinical practice like history taking, presentation of the case and how to approach a patient, etc,. It would be a better experience if it was offline. till now we have learned general examination and history taking.
